Account Creation, Customer Account, Password, and Security Obligations

To access our service, you will create a user account that is secured by the user name (your email address) and accessed through a one-time password (“OTP”) generated for each time your account is accessed.  You are responsible for maintaining the confidentiality of your account, and for all activities that occur under your account.  If you allow third parties to access or use our Website through your username, you agree to defend and indemnify us and our affiliates against any liability, costs, or damages, including attorney fees, arising out of claims or suits by such third parties based upon or relating to such access or use.

Our Website uses an order form to request certain information, products, or services from us, and through which we may receive your contact information (such as an e-mail address).  Contact information from the order form may be used in the collection of individually identifiable health information including medical records, reports, pharmaceutical records, drugs, diagnostic testing and lab work, including HIV-related information, to send orders and information about us to you, or otherwise to communicate with you. 

Health Information Collection and Security

We do not administer any diagnostic, laboratory, or medical tests or measurements.  In creating your report, we and our Website utilize information such as laboratory test results, physical measurements, and individually identifiable health information (including medical records, reports, pharmaceutical records, drugs, diagnostic testing and lab work history, including HIV-related information,) provided to us by third parties, such as third party laboratories or third party providers of diagnostic testing and collection services.  This data is transmitted to us via secure encryption methods and stored using physical, electronic and procedural safeguards that comply with all applicable State and Federal laws, rules and regulations.
